One name for several tissues

Adipose tissue is not a single organ distributed evenly but a set of depots with distinguishable cellular and functional characteristics. The subcutaneous depot sits beneath the skin, while the visceral depot surrounds organs within the abdominal cavity. Cells from these depots behave differently in culture, differing in how readily they take up fat and how they respond to hormonal signals.

They also differ in the density of immune cells present and in the profile of signalling molecules they release. Treating total fat mass as the only quantity of interest therefore discards the information that the distribution between depots carries on its own. The depots respond differently to the same hormonal signal, which is one reason their proportions shift over a lifetime rather than staying fixed.

The drainage difference

Fatty acids released from visceral adipose tissue enter veins that drain into the portal circulation and reach the liver directly. Subcutaneous tissue drains into the general circulation instead, so its output is diluted across the whole blood volume before reaching any organ. This anatomical difference means the liver is exposed to a considerably higher local concentration from one depot than from the other.

The observation is the basis for a longstanding hypothesis linking visceral tissue specifically to hepatic metabolic changes. How large that contribution is relative to other sources of hepatic fatty acid supply remains debated rather than resolved.

Expansion, cell number and turnover

A depot can grow by enlarging existing cells or by recruiting new ones, and these routes have different functional consequences. Enlarged cells become less efficient at storing additional fat and show altered secretion, which affects the surrounding tissue environment. When storage capacity in one depot is exceeded, lipid accumulates in tissues not designed for storage, including liver and muscle.

That accumulation outside adipose tissue is the process most consistently associated with the metabolic changes people care about. It reframes the question away from how much fat exists and toward where the body has been able to put it without spilling into other tissues.

Why measurement is harder than it looks

Distinguishing depots properly requires cross-sectional imaging, which is expensive and not used for routine assessment. Bioelectrical devices estimate composition from the resistance of the body to a small current, and hydration status affects that resistance. Estimates of visceral tissue from such devices are derived through equations fitted to imaging in reference populations, not measured directly.

Simple circumference measurements correlate with imaging reasonably well at population level and much less reliably for one individual. Any single figure describing visceral fat from a consumer device should be understood as a modelled estimate rather than an observation.

What the biology does and does not license

The depot difference explains why two people with similar total mass can differ in their metabolic markers, which is a genuine and useful insight. It does not support any claim that a specific exercise, food or product removes fat from one depot selectively. Depot proportions vary with sex, ancestry, age and hormonal status, which means comparison across people carries limited meaning.

Framing any of this in terms of appearance rather than tissue function is where the science stops and something else begins. Concerns about metabolic health belong with a clinician who can interpret measurements alongside a full history rather than in isolation.
