What a fasting insulin measurement adds that a glucose reading leaves out
Glucose describes the outcome of a control system while insulin describes the effort being spent to achieve it, and the two can diverge for a long time.

A regulated variable hides the work done to regulate it
Blood glucose is held within a fairly narrow band by a control system that adjusts hormone output continuously in response to deviation. Because it is a regulated variable, a stable glucose reading indicates that the system is succeeding rather than that it is unstressed. Insulin is one of the main control signals in that system, so its concentration reflects how much correction the system is currently applying.
A situation in which glucose sits in the usual range while insulin runs higher describes a system maintaining its target at greater cost. That distinction is invisible to a glucose measurement alone, which is the entire argument for measuring the hormone as well.
What the fasting state is meant to standardise
Measuring after an overnight fast is an attempt to sample the system in a defined condition rather than mid-response to a meal. The standardisation is imperfect, since the previous evening's food, sleep quality and physical activity all influence a morning sample. Acute illness and short-term stress move the measurement too, which is why single readings are treated cautiously in research settings.
Repeat sampling on separate occasions gives a considerably more stable picture than any one measurement can. This is ordinary measurement practice rather than a peculiarity of this particular marker, and it applies to almost every blood value reported from a single draw.
The assay problem
Glucose assays are highly standardised and comparable between laboratories, which is a result of decades of deliberate harmonisation work. Insulin assays are not standardised to the same degree, and different platforms can return meaningfully different values for one sample. The assays also differ in how they handle molecules structurally related to insulin, which contributes further to the divergence.
This means an insulin value carries less between-laboratory comparability than most people assume from seeing it on a printed report. Any interpretation therefore depends on the laboratory and the method, which is information a report does not always make prominent.
Derived indices inherit every error
Several widely used indices combine fasting glucose and fasting insulin into a single figure intended to summarise the system's state. Because such an index is computed from two measurements, it carries the variability of both, and combining them does not cancel the noise. The indices were developed and validated in specific populations, and their behaviour outside those populations is less well characterised.
They are useful as research instruments for comparing groups, which is a different task from classifying one individual on one occasion. Reading a derived index as a verdict rather than as an estimate carrying a meaningful margin of uncertainty is by some distance the most common misuse of it.
What the measurement supports
The reasonable claim is that fasting insulin adds information about regulatory effort that glucose alone does not convey. The unreasonable claim is that a particular number constitutes a diagnosis, since diagnostic thresholds are defined by clinical guidelines for specific tests. There is no consensus reference range for fasting insulin comparable to the harmonised ranges used for glucose, and this is why.
Anyone whose results prompt concern needs a clinician who can order appropriate confirmatory testing and read it alongside history and examination. Understanding what the marker represents is genuinely useful; converting it into a self-assigned label is where the usefulness ends.
