What Direct-To-Consumer Lab Panels Can Establish

Ordering tests without a clinical question changes what results mean, because the value of any test depends on how likely the condition was before testing.

Broad laboratory panels are now available without a referral. The tests themselves are the same ones clinics use, but ordering them differently changes how the results should be read.

Test performance depends on prior likelihood

Every test has a rate of false positives and false negatives. Those rates are fixed properties, but their consequences depend on how common the condition is among those tested.

Testing a group where a condition is rare means most positive results are false, simply because there are far more people without the condition available to produce errors.

Clinical testing manages this by ordering tests when something in the history raises the likelihood beforehand. Untargeted panels remove that step deliberately.

Screening has established criteria

Population screening programmes are evaluated against criteria covering whether the condition is serious, whether early detection changes outcomes, and whether the test performs adequately.

Most tests included in broad consumer panels have not been evaluated as screening tools and would not meet those criteria if they were.

Being a valid test for investigating a symptom does not make a test valid for screening people without symptoms. These are different uses with different requirements.

Incidental findings generate their own cascade

A result outside the reference range prompts investigation, which may include repeat testing, imaging or specialist referral, each carrying its own cost and error rate.

Cascades of this kind sometimes conclude with a finding that would never have caused harm, and the intervening process carries real burden.

They can also uncover something genuinely important. The difficulty is that the ratio between these outcomes is unfavourable when testing is untargeted.

Some panels test things with unclear meaning

Certain panels include measurements whose clinical significance is not established, including some hormone metabolites and food-specific antibody measures.

Food-specific antibodies in particular indicate exposure rather than intolerance, and their use to guide dietary restriction is not supported by allergy and immunology bodies.

A result can be accurately measured and still not answer the question it appears to answer. Analytical accuracy and clinical meaning are separate properties.

Where consumer testing is more defensible

Tests with clear thresholds, established follow-up and a condition that is genuinely common are the strongest candidates, since prior likelihood is higher.

Monitoring an already-diagnosed condition between clinical appointments is another reasonable use, because the question being asked is specific.

In every case the results belong in a conversation with a clinician who can weigh them against the wider picture, rather than being acted on directly.
