What Fasting Glucose And HbA1c Each Capture
One measurement records a single moment and the other summarises months, and knowing what each is built from explains why the two results sometimes disagree.

Two tests dominate routine assessment of blood sugar regulation. They measure different things over different timescales, which is why one can appear normal while the other does not.
Fasting glucose is a single point
Fasting glucose measures the concentration of glucose in blood after a period without food, conventionally overnight. It captures one moment.
That moment reflects the balance between glucose released by the liver and glucose taken up by tissues, under the influence of the overnight hormonal state.
Because it is a single reading, it is sensitive to the conditions preceding it. Sleep, stress, illness and the previous evening's meal all influence the result.
HbA1c is a cumulative record
Glucose in blood attaches slowly to haemoglobin inside red blood cells, without any enzyme involved. The attachment is essentially permanent for the cell's life.
The proportion of haemoglobin carrying this attachment therefore reflects average glucose exposure over the lifespan of the circulating cells, which is roughly a few months.
Recent weeks contribute more than earlier ones, since the cell population is continuously renewed. The measure is weighted towards the recent past rather than evenly averaged.
Red blood cells confound the second measure
Because HbA1c depends on how long red cells survive, anything altering that lifespan alters the result independently of glucose.
Conditions that shorten cell survival give cells less time to accumulate attachment, lowering the reading. Conditions extending survival raise it.
Certain haemoglobin variants also interfere with the measurement itself, depending on the laboratory method used. This is a recognised limitation rather than an obscure edge case.
The two can disagree legitimately
A person whose glucose rises substantially after meals but returns to normal overnight can show acceptable fasting values alongside a higher cumulative measure.
The reverse pattern also occurs, with elevated morning readings driven by overnight liver output alongside a cumulative measure that remains within range.
Neither result is wrong. They describe different aspects, which is why clinical assessment uses them together rather than choosing between them.
Thresholds are conventions, not cliffs
The cut-off values separating categories were chosen by expert committees based on where risk of complications begins to rise appreciably in population data.
Risk increases continuously rather than jumping at the threshold, so a value just below a cut-off is not meaningfully different from one just above it.
Interpretation of any individual result, and any decision that follows from it, belongs with a clinician who can weigh it against the full clinical picture.
