Food sensitivity panels: what IgG testing measures and why interpretation is contested
The antibodies these panels detect are produced in response to ordinary food exposure, which is precisely why major allergy bodies dispute their use as a sensitivity test.

What the antibody class actually indicates
The immune system produces several antibody classes, and the one measured by these panels is the most abundant class in circulation. Antibodies of this class directed against food proteins are found in people who eat those foods and have no symptoms at all. Their presence is generally understood as a normal record of exposure rather than as evidence of an adverse response.
Levels tend to be higher for foods eaten frequently, which produces panels flagging the staples of a person's ordinary diet. That pattern is the central reason immunology and allergy organisations have advised against using these panels as diagnostic instruments. A test that flags the foods someone eats most often is describing their diet back to them rather than identifying a reaction to it.
How this differs from allergy testing
Immediate food allergy involves a different antibody class that arms specific immune cells to release mediators rapidly on exposure. Testing for that class is used clinically, and even then a positive result requires interpretation alongside history because sensitisation is not the same as allergy. Confirmation frequently requires a supervised challenge, which is conducted in a setting equipped to manage a reaction.
Coeliac disease involves yet another mechanism and is investigated through specific antibody tests and, where indicated, biopsy. Conflating these mechanisms under one heading of sensitivity is the source of most of the confusion in this area.
Why intolerance is genuinely hard to test for
Many real adverse reactions to food involve no antibody at all, arising instead from enzyme deficiency or from fermentation of poorly absorbed carbohydrate. Lactose intolerance is the clearest example and is investigated through breath testing or through a structured dietary approach. Reactions to fermentable carbohydrates are dose-dependent and vary with what else was eaten, which no single blood test could capture.
Because symptoms are delayed and variable, people find it genuinely difficult to identify triggers from experience alone. That difficulty is entirely real, and it is what makes a test promising clear answers so appealing regardless of what the test actually measures.
The consequences of acting on a panel
Panels commonly flag a substantial list of foods, and acting on the full list means removing a considerable part of the diet. Broad restriction carries nutritional risk and social cost, and it can reinforce anxiety about eating in vulnerable individuals. Improvement after elimination is difficult to interpret, since removing many foods changes total intake, meal timing and expectation simultaneously.
There is also evidence that avoidance of a food can alter tolerance on later reintroduction, which is a documented concern in allergy practice. None of this means symptoms are imagined, and it does mean the panel is a poor instrument for identifying their cause.
What a structured approach involves instead
Investigating suspected food reactions clinically involves a symptom and food record kept over a defined period rather than a single blood draw. Structured elimination and reintroduction, conducted one food at a time, is the approach with an actual evidence base behind it. That process is usually supervised by a dietitian, partly for nutritional adequacy and partly because interpretation requires an outside view.
Investigation also rules out conditions that present with similar symptoms and require entirely different management. Persistent digestive symptoms warrant that clinical route, since several conditions producing them are treatable once correctly identified.
